Issue Summary: The Next Shift
We are moving past the era of "Copilots." For the last couple of years, the conversation around AI has been about assistance—tools that suggest a route, draft an email, or highlight a potential bottleneck in a supply chain schedule. These were advisory tools. They sat on your desk as advisors; they didn't have the keys to the warehouse.
The shift we are facing now is toward "Agents." An agent doesn’t just suggest a change; it executes a series of actions autonomously. It might notice a delay in a raw material shipment and automatically re-route an order, adjust a production schedule, or trigger a purchase order from a secondary supplier without a human clicking "approve" at every step.
The problem is that many leaders are treating these agents as if they are still copilots. They are assuming that because the tool looks like a piece of software on a screen, it carries the same level of risk management as a person-led decision. It doesn't. When you move from an advisor to an operator, your responsibility shifts from "reviewing advice" to "managing a system." If we don’t change our internal processes to account for this jump in autonomy, we aren't just adopting new tech; we are handing the keys to a process that has no brakes.
The Illusion of Decision Support
In many organizations today, there is a dangerous confusion between a tool that helps you think and a system that acts on your behalf. We need to call this out clearly: The Oversight Gap.
An AI agent is not a smarter employee; it is an automated step in your workflow. When the technology was just "advisory," the human remained the primary operator of the process. If the tool gave bad advice, the person still had to make the final move. In that model, the accountability stayed with the human.
When we move to autonomous agents, the human is no longer making a choice; they are setting the parameters for an automated sequence. The Oversight Gap occurs when leadership fails to update their Standard Operating Procedures (SOPs) because "the AI handles it now." If an agent places an incorrect order of 500 units based on a misinterpreted data point, and your team doesn't have a hard stop in place to catch that error before the purchase order hits the vendor, the failure isn't the AI’s. It is a failure of the process design. You cannot manage autonomous outcomes with manual-thinking oversight.
Why We Wait: The Comfort Trap
Why do so many operations leaders hesitate to build these guardrails until something breaks? They fall into The Comfort Trap. It feels easier to let the current "mostly-working" system continue because it’s familiar, even if it's brittle.
We often see a gap between what leadership says they are doing and what is actually happening on the floor. The following table illustrates how this comfort leads to risk:

We are choosing to postpone the hard work of defining boundaries because it’s easier to assume that "the system" will just figure it out. But systems don't "figure it out"; they only do exactly what you told them to do, even if what you told them to do was a flawed command.
What Happens When Agents Fail?
When an autonomous agent fails in a supply chain, the failure isn’t a slow drift; it is often a sudden, sharp spike in cost or chaos. Because these systems can move faster than a human can intervene, a single bad logic loop can result in:
- The Runaway Order: An automated system detects a "shortage" and orders triple the necessary inventory from a supplier because it lacks the context of warehouse space constraints.
- The Ghost Route: A logistics agent reroutes shipments based on a faulty weather data point, sending hundreds of units to a distribution center that isn't equipped to receive them.
- The Contractual Breach: An AI negotiates or accepts terms with a vendor because it was programmed to "maximize efficiency," unknowingly agreeing to lead times that your production floor cannot meet.
These aren't just "glitches." They are the results of removing human oversight from a critical path without replacing it with automated guardrails. The difference between an advisor and an operator is the ability to act independently. If you give an agent the power to act, you must also give your team the tools to stop it.
The Three Pillars of Agentic Oversight
To manage this transition, we have to move beyond "monitoring" and toward true operational control. You don't need a complex IT strategy; you need three clear pillars of oversight that can be audited on any given day.
1. Governance (The Rules)
This is the definition of what the agent is allowed to do without human intervention. It isn't a "suggestion" for the AI; it is a hard limit in the software. For example, an agent may have the authority to re-order materials up to 500 units, but anything over that requires a manual override. You must define the boundaries of its "authority."
2. Veto Points (The Brakes)
A veto point is a mandatory stop in the process where a human must physically or digitally sign off on an action before it leaves your organization's control. In a high-speed supply chain, this might be automated based on risk—if a purchase order exceeds a certain dollar amount or involves a new vendor, the system must pause and wait for a human click.
3. Human-in-the-Loop (The Audit)
This is not about someone watching an AI screen all day. It is about scheduled "check-ins" where you compare what the agent did against what it was supposed to do. This is your version of a layered process audit. You are looking for drift—where the AI’s logic starts to deviate from your actual operational goals over time.
Actioning the Handover: Operational Steps
Moving this from a concept to reality requires immediate action on the shop floor and in the back office. Here is what you can do starting next week:
- Identify "High-Consequence" Paths: Walk through your supply chain map. Identify every step where an automated decision could result in a cost over $5,000 or a delay of more than 24 hours. These are the areas that require mandatory Veto Points.
- Update Your Standard Work: Do not assume your current SOPs apply to autonomous tools. Create a new section for "Automated System Operations." This document must clearly state who is responsible when the AI makes an automated move and what the protocol is if it makes the wrong move.
- Define Failure Modes: Conduct a "pre-mortem" on your current automation. Ask: "If this system fails today, where does the data stop flowing? Who gets the alert?" If the answer is "I don't know," you have an Oversight Gap.
- Establish Hard Stop Limits: Work with your IT or systems team to hard-code limits into the software—maximum order quantities, maximum price variances, and mandatory human approvals for new vendor entries.
Don't wait for a "runaway" event to define these boundaries. If you aren't building the fences now, don't be surprised when the system runs off the road later.
