HFMEA Worksheet Template
An Excel workbook for Healthcare Failure Mode and Effect Analysis: describe the process, score hazards, apply the decision tree, and track actions with outcome measures.
The Process Steps tab records the numbered flowchart of the real process. The Hazard Analysis tab is an Excel Table where each failure mode is scored for severity and probability, and the hazard score and a simplified decision-tree result calculate automatically, with color coding for items that need action.
The Action Plan tab assigns an owner, due date, and outcome measure to each action, with a Days Overdue formula. The Summary tab counts failure modes, shows a severity-by-probability matrix, and charts the decisions. Five example rows for pediatric weight-based dosing are included so you can see the formulas working before replacing them.

How to Use the Template Effectively
- Assemble a multidisciplinary team that includes people who do the work and an advisor who is not part of the process.
- Flowchart the process as it is actually done and record the numbered steps.
- List failure modes for each step and score severity and probability as a team, using the Scoring Key.
- Answer the decision-tree questions for every item scoring 8 or more.
- Write actions with owners, dates, and outcome measures for every Proceed item, and revisit the analysis when the process changes.

HFMEA Worksheet Template Frequently Asked Questions
Can I change the scoring scales?
Yes. The Scoring Key tab shows the commonly used four-point scales, and you can edit the wording. If your organization uses a different scale or threshold, adjust the labels and the threshold of 8 in the Hazard Analysis formulas to match your policy.
What if my process has more than 40 failure modes?
The Hazard Analysis table has 40 formatted rows. To add more, drag the table's resize handle down and copy the formulas in the Hazard score and Decision columns into the new rows.
Is this an official HFMEA form?
No. It follows the commonly published structure of the VA National Center for Patient Safety method but is an independent educational template. Your organization's approved forms and policies take precedence.
