An Excel workbook for management of change: a request form, a register that tracks every change to close-out, and a dashboard of open, overdue, and expired items.
The MOC Request Form tab lists the information a change request should carry, from technical basis to hazard review, document updates, training, and pre-startup review. The MOC Register is an Excel Table where each change is logged and its hazard review, approval, document updates, training, and pre-startup review are tracked as Yes or No.
Days Open, Overdue, Temporary Expired, and Ready to Close calculate automatically, and the Dashboard shows open changes, overdue items, temporary changes past their end dates, and counts by status and type. Four example changes are included so you can see the formulas working before you replace them.

Does this template satisfy a regulation such as OSHA PSM?
No template alone satisfies a regulation. The register supports tracking, but your site's MOC procedure, its hazard review methods, and applicable requirements determine what is needed. Confirm with your safety and compliance professionals and edit the form to match your procedure.
How do the Overdue and Temporary expired flags work?
Overdue appears when a change is not closed and today's date is past its target close date. Temporary expired appears when a change of type Temporary has an end date that has passed and has not been closed. Both use the current date each time the workbook is opened.
Can I add more rows or columns?
Yes. The register has 60 formatted rows. To add more, drag the table's resize handle down and copy the formulas in the calculated columns. You can add columns for your own fields, such as the hazard review reference.
