Preview
List every task or decision down the left column, then name the roles or people involved as column headers. For each cell, choose whether that role is Responsible, Accountable, Consulted, or Informed. Aim for exactly one Accountable owner per row; the preview flags rows with zero or more than one Accountable assignment so the team can resolve the gap before the project starts.
| Role | Meaning | Guidance |
|---|---|---|
| Responsible (R) | Does the work to complete the task | Can be assigned to more than one person or role |
| Accountable (A) | Owns the outcome and signs off on completion | Assign to exactly one person or role per task |
| Consulted (C) | Provides input before the work is finalized | Two-way communication before the decision is made |
| Informed (I) | Notified after the decision or task is complete | One-way communication, no input required |
Use Print / Save PDF to open the browser print dialog and save the finished matrix as a PDF. Use the Word and Excel buttons when you need an editable file for Microsoft Office. Use the Google Doc and Google Sheet buttons when you need a file that can be uploaded or imported into Google Drive.
