Safety Inspection and Audit Pack
An Excel workbook for safety inspections and audits: a weighted checklist, scores by category, a rule that fails the audit on any critical finding, a findings tracker with due dates and verification, and a summary.
The Checklist tab lists items by category, each with a weight (standard, important, or critical), a result (Yes, Partial, No, or N/A), and space for evidence. Points, possible points, and the finding grade calculate. The Scores tab gives a weighted score for each category and overall, counts the findings by grade, and shows the result: any critical finding fails the audit whatever the score.
The Findings Tracker records each finding with its date found, owner, and grade. The due date calculates from the days you allow for each grade, and the status moves from Open or Overdue to Closed - verify and then Verified once the fix is confirmed effective. A Summary tab counts findings by grade and status and shows the on-time closure rate.
An example audit for a metal fabrication plant is included, with 32 items in eight categories.

Critical items count three times as much as standard ones, so the score reflects what matters.
A Critical Failure Rule
Any failed critical item fails the audit, so a high percentage cannot hide a dangerous condition.
Findings With Due Dates and Verification
Due dates calculate from the grade, and a finding is closed only when the fix is verified.

How to Use the Template Effectively
- Edit the categories and items to match the requirements and standards for your site, and set the weights.
- Walk the area with the checklist. Score each item from what you see, what people tell you, and what the records show.
- Read the scores and the result. Contain any critical finding at once.
- Copy each finding into the tracker with an owner and a date found. Due dates calculate.
- Verify each fix at the follow-up, and record whether it was effective.
- Review the summary with the safety committee, and look for repeat findings.

How is the overall score calculated?
Each answered item earns its weight times a result factor: Yes 1, Partial 0.5, No 0. Items marked N/A or left blank are left out. The score is the points earned divided by the points possible, overall and for each category.
How are findings graded?
A No on a critical item is a Critical finding, on an important item a Major finding, and on a standard item a Minor finding. A Partial on any item is a Minor finding. You choose the days allowed to close each grade.
Can I add categories and items?
Yes. The Checklist has 100 rows. Add the category name to the Scores tab, in one of the spare rows, to include it in the scores.
