Lockout/Tagout Procedure and Audit Sheet
An Excel workbook for an energy control program: an inventory that shows which machines have procedures, devices, and a current inspection, a machine-specific procedure form, and a record of periodic inspections with corrective actions.
The Inventory tab lists each machine with its energy sources, isolation points, whether a written procedure exists, whether the devices are adequate, the date of the last periodic inspection, and whether the authorized employees are trained. The next inspection due date and a status calculate: Compliant, No procedure, Devices missing, or Inspection overdue.
The Procedure tab is a form for one machine: every energy source with its isolating device, where it is, how stored energy is released, and how isolation is verified, plus the standard sequence from preparation to restoring the machine. The Inspections tab records each periodic inspection with findings, corrective actions, due dates, and status. The Summary tab shows coverage and open findings, with a chart.
Example data for twelve machines at an illustrative plant are included so you can see the formulas working before you replace them.

How to Use the Template Effectively
- List every machine or group of similar machines that has hazardous energy, with the energy sources and the number of isolation points.
- Record whether each has a written procedure and adequate devices at the machine, and when it was last inspected.
- Write a machine-specific procedure for each machine that needs one, with the people who use it, and add photos of the isolation points.
- Fit missing devices and train authorized employees, starting with the highest-risk machines.
- Carry out the periodic inspections, with an inspector who is not the person using the procedure, and record the findings and actions.
- Review the summary at the safety committee, and close the actions.

How is the status of a machine decided?
In order: if there is no written procedure, the status is No procedure; if the devices are not adequate, Devices missing; if the last periodic inspection is missing or more than 365 days before the as-of date, Inspection overdue; otherwise Compliant.
How do I change the as-of date?
Enter a date in cell B3 of the Summary tab. Type =TODAY() there if you want the status to follow the current date every time the file opens.
Can I add more machines or inspections?
Yes. The Inventory has 40 rows and the Inspections tab 50. Drag the table's resize handle to add more, and copy the formulas in the calculated columns.
