Safety Gemba and Observation Checklist
An Excel workbook for safety walks and short observations: conversation prompts, a log that records what was seen and why, a summary by category with a 95% interval, a Pareto of the reasons behind at-risk work, and the completion of planned leader walks.
The Questions tab holds twelve conversation prompts about the work, barriers, tools and time, hazards, energy and machines, stop-work, and how the system responds, with space to note what you heard. The Observation Log records each observation with the area, task, category, result, the main reason if the work was at risk, and any action.
The Summary tab calculates, for each category, the number of observations, the number at risk, the share safe and its 95% Wilson interval, and counts the reasons behind at-risk work with a chart. The Walk Plan tab tracks planned and completed leader walks by month with completion percentage.
Example data for a month of sixty observations are included so you can see the summary working before you replace them.

How to Use the Template Effectively
- Plan the walks: who, where, and how often, on the Walk Plan tab.
- Take the prompts on the walk and have a conversation with the person at the job.
- Record each observation, its result, and the main reason if the work was at risk.
- Fix urgent issues on the spot, and log the follow-up.
- Review the Summary monthly: the categories with the lowest share safe and the biggest reasons.
- Act on the reasons in the system, and tell people what changed.

How is the interval calculated?
The Summary uses the Wilson 95% interval for the share of safe observations in each category: (p + z squared over 2n, plus or minus z times the square root of p times 1 minus p over n plus z squared over 4 n squared) divided by 1 plus z squared over n, with z of 1.96. It stays sensible for small counts and for shares near 0 or 100 percent.
Should I use the safe percentage as a target?
No. A target for the share of safe observations teaches people to look safe when watched. Use the reasons behind at-risk work to fix the system, and the share safe only as a rough trend with its interval.
Can I add categories or reasons?
Yes. Edit the category and reason lists in the data validation of the Observation Log, and add the same names to the Summary tab so that they are counted.
